Each year, nearly three million people die worldwide as a result of occupational accidents and work-related illnesses. This alarming statistic from the International Labour Organization (ILO), published in 2024, represents a 12 % increase since 2000. It shows that despite technological and regulatory advances, workplace safety remains a critical global challenge.

In occupational safety management understanding the difference between “hazard” and “risk” is essential to prevent these statistics from continuing to climb. A hazard is a condition with the potential to cause harm whether it involves a chemical substance, a hazardous activity or heavy machinery. However that hazard may manifest in different ways depending on three main factors that determine how it evolves into a real risk for workers.

The anatomy of hazard: Source Situation and Circumstance

Hazard at the source refers to the actual object or situation that can cause harm, such as unguarded machinery or a flammable substance. Hazard in the situation relates to the conditions under which that object or scenario appears for example a poorly maintained machine or prolonged exposure to a harmful substance. Finally hazard in the circumstance refers to the specific conditions in which the work is performed such as the surrounding environment or aggravating factors like excessive heat time pressure or unsafe behaviors.

It is precisely between hazard and risk that the possibility of an accident arises. To reduce the risk of an accident organizations must understand and manage these three aspects of hazard. That is where control layers come into play they serve to mitigate hazards in their various forms and prevent them from becoming fatal realities.

James Reason’s Swiss Cheese model A contemporary systemic approach

The idea of control layers is based on the principle that we cannot rely on a single barrier to prevent accidents. The “Swiss Cheese Model”, developed by James Reason in 1990 and still widely recognized today, illustrates how accidents occur when multiple layers of defense align through “holes.” These holes symbolize various failures or unsafe conditions that may open and close randomly.

The model distinguishes between “unsafe acts” direct actions leading to accidents and latent organizational conditions that may go unnoticed yet contribute to such unsafe acts. In 2024 this framework continues to be adopted across sectors including transportation public health and cybersecurity and has been used to analyze public health crises such as the COVID‑19 pandemic.

Each control layer acts as a defense against hazards at the source situation and circumstance creating a more robust safety system. By focusing on interactions among different levels of failure the model promotes a systems‑oriented approach to safety encouraging investigators to look beyond individual errors toward underlying organizational processes.

Modern control layers Beyond traditional measures

Contemporary control layers encompass measures that go well beyond traditional approaches. Engineering measures such as physical guards on machinery containment systems and isolations for hazardous materials remain fundamental. Proper employee training empowers workers to recognize and address hazards in their various manifestations while periodically reviewed standard operating procedures ensure work situations and circumstances are constantly monitored and adjusted.

The proper use of personal protective equipment (PPE) minimizes hazard in specific work circumstances. By combining these layers we can protect workers both from inherent source hazards and those arising in situations and conditions of the work environment.

Critical controls The backbone of modern prevention

Among control layers critical controls are key and their management has evolved significantly in recent years. These controls are those that if they fail carry the highest potential to cause a serious or fatal accident. Therefore identifying these critical controls and ensuring they are continuously monitored is essential.

According to 2024 best practices effective critical risk management requires that risks are identified documented communicated and implemented clearly. Robust controls must be identified before relying on procedural controls and responsibilities must be clearly assigned for each identified risk and control.

One example of a critical control might be a ventilation system in a chemical work area. If this control fails the circumstances (a confined space) amplify the hazard risk from the source (chemical substances). Therefore continuous monitoring and flawless operation of this critical control are imperative to ensure safety.

Critical controls should not be treated superficially they involve deep analysis of hazard sources and their variations in work situations and circumstances. They require a deep understanding of how these variables can align to produce an incident.

The reality of the numbers Why prevention is urgent

The most recent data highlights the urgency of implementing robust control systems. Of the 2.93 million annual work related deaths a staggering 90 % are caused by occupational diseases (2.6 million) not immediate accidents. That means most fatalities result from prolonged exposure to hazards that could have been controlled through adequate protective layers.

Disparities are significant men face a mortality rate of 108.3 per 100 000 workers compared to 48.4 per 100 000 for women. The Asia Pacific region accounts for nearly 63 % of global work related deaths due to its large working population.

In the United States fatal workplace injuries in 2022 reached 5 486 while non fatal injuries in private industry totaled 2.8 million recorded cases. In the European Union there were 3 347 fatal workplace accidents in 2021 in addition to 3.2 million non fatal accidents resulting in at least four days away from work.

Anticipatory mindset The key to mitigating risk in the 21st century

Effective prevention requires adopting an anticipatory mindset that goes beyond traditional reactive approaches. Many companies still operate reactively responding to incidents and near misses only after they occur. However a safer approach involves anticipating hazards and risks through five fundamental strategies identified by experts in 2024.

  1. First it is essential to identify gaps in the safety management system analyzing where vulnerabilities exist and developing improvement plans with specific timelines.
  2. Second regularly verify the assurance framework ensuring “work as done” aligns with the safety management system.
  3. Third all review recommendations should be closed within established deadlines with regular progress reported to the board.
  4. Fourth there should be an effective and appropriate system for critical risk management where risks are clearly identified documented and communicated.
  5. Fifth it is essential to measure framework effectiveness through regular assessments and performance metrics.

This mindset requires acknowledging failures as inevitable in any system anticipating where and how they may occur and preparing to mitigate them. Human error is also inevitable and rather than punishing mistakes we should create an environment where they can be detected and corrected quickly before they escalate into serious incidents.

Variability and accidents are realities in work situations that don’t always go as planned. We need to be prepared to handle the unexpected creating robust defenses that absorb such variations without compromising safety.

High‑risk sectors Lessons learned

European data reveal that sectoral differences are significant with construction and manufacturing reporting the highest numbers of both fatal and non fatal work incidents. These sectors require stringent safety measures due to the physical hazards involved in daily tasks.

Small and medium sized enterprises (SMEs) face greater risks due to limited resources for safety training and equipment. Regional disparities are also apparent with some Eastern European countries showing higher accident rates while Western European countries with stronger safety regulations tend to see fewer fatalities.

Building a resilient safety culture for the future

Creating a resilient safety culture requires constant commitment to anticipating controlling hazards risk and variability. It is critical that leaders and employees understand the dynamic nature of the work environment and the importance of adopting a holistic approach that includes all forms of control.

An effective safety culture includes ongoing dialogue about hazards and risks encouraging all levels of the organization to identify and report hazards and shifts in the work environment. Training about specific hazards through regular sessions helps employees recognize hazards at the source in the situation and circumstance and take appropriate action.

Continuous monitoring and improvement of controls ensures that critical controls are regularly reviewed and updated to match changing processes and working conditions. This includes not relying on mere “check the box” mechanisms but focusing on identifying what isn’t being done and filling gaps through effective audits field interactions and senior leadership observations.

Legal and Ethical Responsibility The new paradigma

In 2024 corporate accountability for workplace safety reached a new level. Boards and directors now have a legal duty under health and safety laws to ensure the company meets its obligations known as the “duty of care.” More significantly executives now face imprisonment risk for industrial manslaughter and recent court rulings appear to expose directors to greater personal liability.

This means safety management is no longer merely operational but a strategic responsibility requiring attention at the highest organizational level. All risks and controls must be reviewed at regular intervals with immediate follow up after significant incidents processes or improvement notices.
